See Pain Doc: Generally tolerance or "immunity" to pain meds comes with intermittent high doses of these medications rather than using a constant low dose like what you would get from a prolonged release or extended release medication. Switching to a long acting or using smaller doses more often can cause some withdrawal like symptoms which requires some expertise to manage. You need to see pain doc.
