Not unexpected: The range of the awake heart rate of a 4yo is 60-140. The addition of fever generally increases the rate by 10 beats/degree/min. The respiratory rate also increases by ~5 breaths/min/degree. A kids heart is much more tolerant of rapid rates than adult & with some illnesses we will see it in the 180+ range for a couple days.
