Tiny tiny breaks?: Skin has many tiny openings when looked at with a microscope. Sometimes germs get in that way. Also, germs live on our skin all the time, but abscesses form only when some of the "unfriendly" germs are trapped under the surface and multiply. Occasionally, germs may get to the skin from within the body, and then start an abscess just under the skin.

Microscopic: Just because you can't see the break doesn't mean there wasn't one. Subcutaneous cysts can also get infected. The infection could be seeded from another part of the body, for instance access or cellulitis of the leg can be from a break of skin between the toes.
