Not always: Sometimes it is obvious that a patient is sexually active from findings on a pelvic exam, such as sperm in the vaginal discharge or the finding of an std. Sometimes there are findings on exam that suggest sexual activity, but are inconclusive, such as a torn hymen. For most women, if they do not admit to being sexually active, it would be impossible to be certain either way.
