Rarely: Keep in mind that there is no such thing as 100% in life or in medicine. For every test we do, there is some chance (which may be very, very small) that the result will be incorrect.

Yes: Most are quite accurate but it is possible to get a false positive or negative depending on wear the tissue is bippsied.
