Lots of things: Short list: musculoskeletal pain, gallbladder stones or inflammation, liver inflammation or infection, pancreatitis, pneumonia. If your pain is severe, associated w nausea/vomiting, fever, or if it lasts several days i would have your primary care doctor take a look.
