Venous stasis/lumbar: In advanced pregnancy, it's common to get low back pain/low body pain.Baby may be sitting against inf. Vena cava slowing blood return, causing blood to pool in lower body causing heaviness, pain, swelling, etc.Also, baby pulling abdomen forward pulls lumbar vertebra out of whack, too, causing back & lower extremity symptoms.Changing positions frequently and gentle osteopathic manipulation may help.

Time 2 see OB doctor: The causes may be complex. The step that should be taken in simple. A woman near term with pain is best seen on labor and delivery for examination of her and monitoring for contractions and the fetal status. Calling the office to let them know a patient is on the way is nice but not necessary if there is so much pain.
