Premenopausal Acne: This is a common phenomenon precipitated by predominance of testosterone hormone prior to a period. This may be controlled with oral medications, including oral contraceptives (oct's). Nevertheless, you are 40 years old and there is a concern that use of oct's may result in a thromboembolic event, especially if you are a smoker. Therefore, it is essential that you are evaluated by a physician.

Difficult: You don't give your age. Acne before your period is usually hormonal acne. If you have been mensruating for 2 years continuous cycling of the pill would stop your problem. Sometimes we have added small doses of estrogen just before the expected breakout time and that has helped too. Others have tried with lesser success antibiotics.
