Other options.: Use of an antibiotic to which the bacteria are resistant will result in treatment failure. This is why it is so important to get a urine culture & perform sensitivity testing to determine which antibiotic will work. In an elderly patient with renal failure, the dose may need adjustment depending on the medication. Many options do exist, so consult with your physicians.

Fix underlying prob: The most effective way to treat a UTI is to fix the underlying problem causing urine backflow, retention, or both. Anatomical anomalies or else weakened bladder or an enlarged prostate can be the cause of utis. Addressing these issues, along with the appropriate abx choice , will help rid a pt' s uti.

Lots: First, realize that close to 20% of elderly women will have bacteria in the urine but not be infected. Be careful not to over treat bacteria in the urine.
