What injury of the knee could lead to pain behind the kneecap and the inability to fully extend the leg?

Several things: Cartilage wear behind the knee cap called chondromalacia, or degenerative arthritis, quadriceps tendinitis, patella tendinitis, and possibly an anterior horn meniscus tear. If it was a impact injury a patella fracture or bone contusion could do the same.
