See Podiatrist: He will evaluate and examine and probably x-ray and provide treatment depending on cause.

See a doctor: You could be dealing with a bone spur. See a doctor for an x-ray and an exam.

The question : Is slightly ambiguous. There is no joint under the big toe. So, all i know is your big toe hurts and possibly at a joint. Not sure if that is the joint of the big toe or the one just before it. Either way, sounds like you should get evaluated and an x-ray.

See Podiatrist: Several possiblities. Among them, bursitis, fracture, arthrithic condition or infection come to mind. A podiatrist will examine and x-ray and provide diagnosis and treatment.
