Clinical setting: This depends a lot on the setting. In a newborn, high bilirubin is usually transient, but may indicate hemolytic disease related to maternal antibodies. In young adults, often Gilbert's disease and usually insignificant; in an older adult, it often reflects blockage of bile ducts by stone or tumor.
