Need2CoordinateCare: In general, surgery plays very little role in the rx of stage 4 breast cancer; however, stage 4 is a broad category. Your loved-one has many chronic illnesses, any one of which may compromise her rx. She needs her primary care doctor and oncologist to coordinate their care based upon these issues, with all parties determining what's best for her.

No & yes: Surgery for stage 4 not helpful, more so in someone with so many medical problems. If appropriate then your oncologist may recommend a trial of hormonal therapy (tamoxifen, arimidex, (anastrozole) etc). Good luck.
