Too little info: Too little info here...Could be many things from trauma to fungal issues...You need to see your family doc or a podiatrist for a good history and physical exam.

Could be: Fungus or could be blood from shoes being too tight, or "runners toe". This is something that on clinical exam a podiatrist can tell you in less than a minute.
