Several causes: Arthritis, tendonitis and bursits. You need a physical exam to make the diagnosis.

Several poss. causes: Arthritis is common. Bursitis, tendonitis, small tears from overuse (i.e. Unnoticed injury), inflammatory disorders, intestinal injury (air in abdomen!), shoulder girdle prob. (poorly functioning ac or sternclavicular joints), etc. Posterior rib pain is also mistaken for 'shoulder pain' as it is behind the shoulder. A good history & phys, including osteopathic exam might help find & treat the cause.
