Many possibilities: Structural:cord accidents (true knots in cord),placental abruption, premature placental failure;Infection (viral, bacterial);chromosomal defect;Maternal health issues. Many times the true cause is not evident on initial observation, so a full autopsy by a pathologist with experience in this area may provide useful information.
