Many pieces: A mental health exam ought to contain your presenting problem & its history, past psychiatric history, present & past medical illnesses, medications you're on, your family history, social history, review of systems, and a mental status exam. The latter is quite detailed and includes your affect, attitude, appearance, thought process, memory function, presence of hallucinations or delusions, etc.
