Depends on state : Many states now require some paternity information. However, obviously occasionally a woman may have more than one partner, or may not remember who the potential father is. In other cases where abuse or rape were involved the patient may not want to divulge the father's name. The only issue you should be really aware of is that it is difficult to obtain child support if you don't name a father.
