Usually: Any internal bleeding within the abdominal cavity tends to irritate the lining of the cavity, simulating peritonitis, and causing significant pain. On rare occasion, a "contained rupture" of the spleen may occur which is not painful, only to "open up" days top weeks later.

Yes they are painful: They present with pain in the lt. Upper quadrant of the abdomin, and lt. Shoulder pain.
