What's a truly informed consent form for someone with mood disorders?

Informed consent: First there is a discussion of your health issue, and recommendation for treatment. Alternate treatments and questions from patient will be answered. Your support, family and friends will be involved. The process is more about the risk/benefit analysis, and making the best decision for your health. Its not about your level of information retention. Make sure your family is with you.
