They can. Should???: Almost 80% of all twins in the United States are delivered via cesarean section nowadays. There are several good reasons for this upward trend: extremely few obstetricians skilled in breech delivery/extraction of the second twin as singleton breech deliveries have stopped since 2000, increased risk for perinatal injury/death for the second twin in vaginal birth, 4-10% risk of stat cesarean section.

Yes: Twins can be delivered vaginally but a C/S may be recommended for many different reasons.
