Very Common: Many people with gallbladder attacks describe the pain "radiating" around into the right-upper back. This is due to irritation of the liver and diaphragm which extend around to this area. Occasionally, people note a constant dull pain in the back, as well. The pain may also extend to the shoulder area, since the nerves that innervate the diaphragm also go to the shoulder.
