Many: Large ovaries can be caused by either simple functional cysts which come and go with the cycle. Or benign tumors which can be more frequent. Also malignancies will develop solid tumors. Cysts filled with old blood can be endometriomas which cause enlarged ovaries. Also polycystic ovary syndrome will have bilaterally enlarged ovaries.
