PE tubes: You've got to restore the ventilation and drainage mechanism- i.e., the eustachian tube's competency. That usually means time and/or tympanostomy tubes.

??PAIN??: I doubt that any toddler has significant pain with glue ear.That is defined by the presence of fluid in the middle ear space. You only get pain with significant pressure like you have with infection.You can get a muffled feeling like when you need to pop your ears after a flight.Toddlers can have fluid for months without symptoms. Either way, ibuprofen can help with discomfort.
