Absolutely!: Even if there are conscious choices initially about purging, an eating disorder is definitely not an "intentional" condition. And, it has to do with more than just body image. Self-esteem, mood, relationships, personality, anxiety, are all factors -- and more! if you even suspect an eating disorder, get expert evaluation and assistance. Quick!

It is unintentional: I don't think people intentionally develope eating disorder/anorexia. Anorexics often have intense fear of wt gain even they may already be underweight--it is an irrational fear. Despite underwt, they diet, exercise to excess, or use other method to lose wt--such as forced-vomitting after meal etc... Treatment is difficult and require meds and behavior therapy. Only way : An eating disorder is a neuropsychiatric problem. No one wants to be sick or disordered. It just happens (for many, many reasons none of which are voluntary). Rarely, people with münchausen syndrome or histrionic disorder will fabricate symptoms for gain. But his is rare, and usually more medically oriented diseases are feigned.
