Tight foreskin: Phimosis or tight foreskin can cause problems during sexual intercourse as well as it cause para phimosis with painful swelling. Also prone for infection as it cannot be retracted easily. Circumcision is the treatment.

Not likely: But see your physician if the foreskin is causing you symptoms of pain or if you cannot retract it to expose your glans.

Troublesome: Tight foreskin (phimosis) makes retraction uncomforable or painful especially during sex. Hygeine is difficult, or impossible if foreskin is too tight to retract. He should consult with a urologist because it won't get better on its own. Circumcision is usually the best option for an adult. Steroid cream may give some temporary relief, but phimosis after steroid cream usually recurs in adults.
