Likely psychological: Since bulimia is an obsession/compulsion to eat and then throw up everything, the patient's body doesn't retain food to digest. The "withdrawal" would be the psychological stress of trying not to succumb to the strong obsessive/compulsive forces. There may be some physical symptoms if the patient eats a lot, and actually keeps it in and digests it, since he is not used to digesting that much food.
