They turn their head: Belly sleeping was common between ww1 and 1992 in the mistaken belief it was safer for kids when they spit up. Studies in the 80's-90's showed this wrong. Crib death rates have declined 40-50% since back sleeping became the new standard.In the same period we have not seen an increase in choking deaths. Barfing infants turn their heads & recover well.
