Sometime: Bullous emphysema may be painful if the bullae expand in size and stretch the pleura and cause pain. If a bulla gets infected there may be pain and finally if a bulla bursts there will be pain from the resultant pneumothorax.

Not typically: In general emphasema, including bullous emphasema, is uncomfortable and people feel short of breath but it is not usually painful.
