It means a woman loses more than a liter of blood after c-section or more than half a liter after vaginal birth. Physicians have little time to act decisively to stem the hemorrhage before it becomes life-threatening. Blood transfusion, surgery, uterotonic meds, hysterectomy and intensive care!
