Many possibilities.: If each whitehead is emerging from a hair follicle it's probably a folliculitis, which requires antibiotic treatment; tell him to see a dermatologist before he starts getting hair loss. If they're in between hair follicles they could be small epidermal cysts, which a dermatologist can remove. There are other possibilities as well, but he needs to be examined.
