Documentation: Bruises are very common in the elderly, due to fragile skine, and can occur spontaneously (someone on blood thinners) or with routine hands on care. Nursing homes are so well regulated now, the nursing staff is required to document all changes of a patients skin. Staff abuse is rarely a cause for bruising, however if this is suspected discuss with the administrator and the director of nursing.
