Yes: During its active years smallpox in general had a 30% case fatality rate, highest in children and older adults. Some subtypes with malignant or hemorrhagic smallpox approached a 100% fatality rate.

Smallpox = deadly: Smallpox from Variola Minor had a death rate of 1% or less. 90% of unvaccinated smallpox cases fell under the category of “ordinary Smallpox”. The overall fatality rate for Smallpox was 30%. Malignant (AKA flat) Smallpox made up 5 to 10% of cases, was usually fatal & mostly effected children. Hemorrhagic Smallpox, which is usually fatal, made up 2% of cases & mostly affected adults.
