Yes! But very rare.: A boy can have more than two testicles, a condition called polyorchidism. This is extremely rare, with a 2009 meta-analysis documenting only 140 histologically confirmed cases in the literature. The exact cause is unknown, but some event in the fetus at 8 weeks causes the developing genital tissue to divide abnormally. Treatment is controversial, with some having the extra testis removed.
