If an optometrist finds a vision impairment must they add it to your chart?

A requirement: Good physician practice is to place an account of all abnormal findings as well and pertinent negatives on the patients record. This is helpful for monitoring and follow up. It sounds as if you are worried that someone may be looking over his shoulder - perhaps you do not want to get that examination?
