Heparin, warfarin...: The most common two are Heparin and warfarin. Heparin is typically continuously infused in a hospital setting and can be given twice a day at home via subcutaneous injection. Warfarin is a pill that is taken daily and requires monitoring of a blood level. There are others, but these are the most common by far.
