Life or death yes: A fracture already hurts but if this is a life or death situation yes. Yes you could make a fracture worse, but this also depends where you are. In a controlled environment like the hospital there are other ways.

Yes: Paramedics are trained to provide emergent stabilization of a critically ill patient and to transport the patient as quickly and safely to the nearest hospital. They are trained in spinal precautions. If someone needs CPR, not performing CPR would likely lessen the patient's chance of survival.
