Not pregnant?: Transvaginal ultrasound detects pregnancy around the time your HCG level is about 1500; so well after a pregnancy test becomes positive. This is generally about 1 -2 weeks after a missed period.

Early pregnancy: Transvaginal sonography can detect a pregnancy- intrauterine gestational sac- as early as 5-6 weeks after last menstrual period or 3-5 weeks after conception. You need a positive pregnancy test first.

About 6wks: Confirmation of pregnancy is evident at about 6wks gestation, though earlier signs of pregnancy can be seen closer to 4wks. However, urine and blood tests are much more sensitive to detect early pregnancies.
