Couple of things: Anything from a slipped disc to arthritis to thoracic outlet syndrome (a compression of nerves from bone, muscles or even veins/arteries) can cause this. I would start with your primary physician.

See below: The pain could be coming from the disc, an irritated cervical nerve, thoracic outlet, etc. The pain could originate in the shoulder and be radiating to the neck and chest.

Several possible : Reasons including neck problems like a disc herniation leading to a pinched nerve, a shoulder problem, certain tumors of the upper lung known as pancoast tumors, heart related issues such as angina to name a few. Because this may be related to a heart problem , see your doctor.
