What are solutions for the currently high cost of hospital care?

Various measures: Health care expenditures are skyrocketing. My opinion is that it needs a complete overhaul, which may not be possible politically. Most of the physicians, nurses practice what is called defensive medicine. This entails to order more and probably unnecessary tests adding up to the cost. Even patients also go to er to er getting the same tests done wasting dollars. Regulating costs in all fronts.
