What are some lists used to ensure patient safety in the operating room?

Checklist: There are lot of measures taken to prevent errors in the operating room such as timeouts, check list that the chief scrub nurse goes thro. This prevents wrong part of the body being operated upon, whether instrument count is accurate or correct number of gauzes used etc.

FACS: The american college of surgeons or acs has a website with specific time out lists (facs.Org) this is a national organization over 100 years old.
