No intrinsic factor: B12 in the diet usually binds to intrinsic factor in the stomach and then gets absorbed by the small bowel. In pernicious anemia, there is low or no intrinsic factor being made by the stomach cells. As a result, there is gradual deficit of B12 stores and eventaully as all the B12 is gone the new red blood cell production is affected.
