Meds, possibly Botox: There are topical medications (in cream form) that can be used to help fissures heal; diltiazem is prob most commonly used. For patients who fail to heal with this, injection of Botox to temporarily paralyze the internal sphincter can produce healing. I try to avoid sphincterotomy in women due to the higher risk of continence impairment (higher than in men).
