Maybe: There are several things that could cause the knot in your scrotum. Varicocoels and spermatocoels are the most common as well as hydrocoels. Go see your doctor to determine if there is a problem.

Go to Doctor: You need to see a urologist for an exam. Although most scrotal masses are not cancerous some are and if it's attached, it definitely needs an exam.

?Poss. cyst or tumor: Spermatocele or epididymal cyst on top of tesis should have a slightly soft feel and should be uncomfortable or tender to palpation. Tumors are hard and usually non tender. Please see a urologist if there is any question of a tumor. Progonosis for testis tumors is usually excellent and sooner it is treated the better.

Don't know: I don't know what you mean by a "knot". I assume you mean a painless, firm scrotal mass. If so, you need a physical exam and a scrotal ultrasound.
