Pretty specific test: As in from a statistics perspective, if this test is negative is good for excluding the disease, in this case thromboembolic disease like from a pulmonary embolus(lung blood clot). True it isn't perfect, but realize there are many causes for shortness of breath and cough that aren't blood clot related. Is there a reason why you suspect a blood clot, so much?

PE concern: D-dimer is 97%-99% true negative. Factors that interfer: factor XIII deficiency,tube not properly filled,lipemia, late after the event ( > 10 days) and medications like anticoagulants. If Wells score 10, previous PE, malignancy ,coughing blood, signs of DVT ( leg swelling) and high concern for PE is low and d-dimer is negative, PE unlikely. If concerned get V/Q or CT angio.
