Yes: The mucus plug is not a very reliable indicator of cervical dilation. Not every women knows when it comes out. Its easy to miss. So it is possible to dilate and efface without seeing the mucus plug. Best wishes!

Possible: The cervical mucous is thick and sticky so it will try to stay in the cervix. However, as you dilate the mucous will inevitably begin to pass out of the vagina. You could notice large clumps of mucous but you may only see an increase in the amount of vaginal discharge as the mucous plug goes away.
