Serum sickness: Highly unlikely. I am not aware that serum sickness reactions portend reactions for other family members which we sometimes see in patient with ige mediated reactions to medications- though the studies are equivocal.

Usaully gut!: Inflammation of the GI tract, usually subclinical, is present in all the seronegative arthopathies and IgA is frequently elevated. It is mostly made from lymph tissue ligning the small intestine, called peyer's patches. It is also elevated in regional enteritis and crohn's of the small intestine. In an older person, multiple myeloma must be excluded.

Depends on values: Elevated IgA level might just represent an adequate response to an infection. If this is the case, when the infection is gone, the levels would come back to normal. Less common, they could be extremely high and might indicative of a proliferative disorder of the cells that produce the iga.
