Lots of stuff there: On the left side are the stomach, spleen, diaphragm (muscle for breathing), ribs, a part of the colon (large intestine), the tail end of the pancreas, etc... Pain can be from the ribs and their cartilage, a swollen spleen, stomach acidity/gastritis/reflux, muscle strains in the abdominal muscles or diaphragm, excess stool or gas in the colon, pancreatitis, a cancerous tumor, etc....

Many causes: This problem mandates a real-time (face-to-face) meeting with a doctor. A history has to be taken along with a examination as well as labs and other tests. Only after all this will your doctor be in a position to tell you what's wrong and what needs to be done to help you.

Dr. Martin Raff

Many possibilities: This could be from something irritating the diaphragm from above or below, from the rib or the costochondral attachments, an enlarged and tender intraabdominal organ like the spleen, or could be referred pain from elsewhere. Answering your question accurately would require a full exam. Good luck.

What causes Pain below rib cage? Hurts when I breathe

Doctor can evaluate: One has to find out what is causing pain in order to decide on treatment of it. Things that are under the ribs include the breathing muscles (diaphragm and rib muscles), stomach, liver, spleen, gallbladder (which might have gall stones), part of small intestine, part of large intestine, part of the pancreas, etc... If pain comes with breathing, the source could also be ribs and their cartilage.
