Worrisome: He needs to see an ophthalmologist for both these problems. He needs to see someone now. Not focusing and a white reflex is very concerning and take whatever route you need to have him seen. Talk to your provider to get help. All the best and I hope he does well.

Yes: This is serious. A full evaluation is in order. Many of these eye cancers can be cured. This may also be something else entirely and easily managed.

See EyeMD: There are a number of possible reasons for a white reflex in the pupil of a child. Cancer is one of them. You should make an appointment with a pediatric ophthalmologist for evaluation.
