Yes it can: Damage to the testicles can be from direct trauma or lack of blood supply. In both cases the testicles tend to shrink. The testicles can be soft and smaller than normal since birth as well. The patient needs to be check by an specialist.

Usually that happens: Due to the use of anabolic steroids, and it's called testicular atrophy... Or primary hypogonadism or a pituitary issue. This needs to be seen by endocrinology.
